CAMBRIDGE DENTAL & VISION FUND MEMBERSHIP PORTAL IS HERE!

We are pleased to announce a new Member Portal where members can view their information, dependent information, vision claims history, eligibility dates, dependent statuses, download vision claim forms, and NOW Pay COBRA and Retiree Premiums Online! 

Members who already registered and created an account can simply sign-in. Those who are new to the member portal can enroll by creating a user account. You can review the instructions to enroll in this step-by-step guide.

Get Started

Information needed to create an account:

  • Member Last 4 of Social Security Number
  • Member Date of Birth
  • Member Email Address – Already Registered on File with Fund

Important Information:

Before creating an account, please confirm with the Fund that all your information on file is correct. Retirees can confirm their information on the recently mailed Retiree Continuation Coverage Renewal Notice Form.

Active Employees or Retirees who wants to change or update their information can email the Fund at info@cdvfund.org or call 617-354-1110.
