Before obtaining vision services, please verify eligibility for you or your dependent(s), by calling the Fund Office at 617-354-1110 or email info@cdvfund.org.
Dependent Children Verification
Eligible dependent children are covered up until under the age 26 – coverage ends on 26th birthday, provided they meet the eligibility provision requirements – see “Dependent Children Provision Summary.” Dependent children that do not meet the provision requirements, but are full-time students are covered until the age of 25 – coverage ends on 25th birthday; and totally-disabled dependent children can remain your coverage regardless of age.
